ECG vs TMT Test: Which One Do You Actually Need?

An ECG records your heart at rest. A TMT records it under stress. They measure different things — here's which to choose.

Walk into a diagnostic lab in India and you'll see both an "ECG" and a "TMT" on the menu. People often pick whichever is cheaper without realizing they answer different questions. Here's the difference.

ECG (Electrocardiogram)

A resting ECG records your heart's electrical activity for about 30 seconds while you lie still. It shows rhythm, rate, electrical pathway, signs of past heart attacks, and chamber enlargement.

  • Catches: Arrhythmias, conduction issues, past damage, chamber strain
  • Misses: Blockages that only show up during exercise

TMT (Treadmill Test / Stress Test)

A TMT records your ECG while you walk on a treadmill that gradually increases in speed and incline. The point: stress the heart and watch for changes that don't show at rest.

  • Cost: ₹1,500–₹3,000 typically
  • Time: ~30–45 minutes
  • Catches: Exercise-induced ischemia (early coronary blockages), exercise tolerance, abnormal blood pressure response
  • Misses: False negatives in early disease; can't be done if you can't walk

So which should you start with?

For most people with no symptoms, start with an ECG. It's cheap, fast, and rules out the most common silent problems. If you have a strong family history, symptoms on exertion (chest discomfort, breathlessness, sudden drop in stamina), or known risk factors like diabetes, a TMT is worth the extra time and cost.

The smart sequence

ECG first. If anything's flagged, or if your doctor wants to see how your heart behaves under load, then TMT. Skipping the ECG and going straight to TMT is common but inefficient — and TMT done without a baseline ECG is harder to interpret.
